In most cases, the following PPO policies are in network with our office. We will verify benefits prior to each patient’s appointment. Should a policy be considered out of network we will inform the patient of the network’s status prior to their appointment. As a courtesy, we will file a claim to out-of-network insurance for direct payment to the patient.*please note- We are only in network with the listed HMO policies.
